The DRIVINGHEALTH^® INVENTORY including UFOV^® is offered in two modes:
1) on a fee-per-use basis, and 2) as a fixed-price annual software license that permits a single copy of the program to be used an unlimited number of times.  

Under the fee-per-use mode, the DRIVINGHEALTH^® INVENTORY may be purchased for US$10.00 (pre-paid) per screening.  This option is ideal for low-volume users.  Alternately, a license allowing unlimited use of the screening program, distributed on portable media that may only be used on one computer at a time, may be purchased for $500.  If a user anticipates more than 50 driver screenings in a year, the fixed-price, unlimited use option is more economical.

Two delivery options for the DRIVINGHEALTH^® INVENTORY including UFOV^® are available, online and portable media.

Online option:  Fee-per-use customers who log in with a current (pre-paid) account can conduct driver functional screenings at any location on a PC with a high-speed Internet connection, provided that a small (~8 Mb) "player" is downloaded first. This player may either be downloaded to:

     a) the local hard drive, or

     b) to a user's own USB/flash drive, that is then inserted into a USB (2.0) slot to          perform a screening.  

In either case, the user's account is credited with a pre-paid number of screenings, then is debited as each screening is completed, via the Internet connection.

We will ship a USB/flash drive to you with your pre-paid number of program uses, or the unlimited-use version of the program, already loaded.  A US$ 19.50 charge for the cost of each USB drive purchased, which includes (domestic) courier delivery, will be applied for customers whose initial order (per drive) is less than 50 uses.

Uses of the DRIVINGHEALTH^® INVENTORY including UFOV^® may be purchased online via PayPal, or customers may send a (cashier's or company) check to establish or to increment their accounts.  Please navigate to the 'Order Software' page and click the 'purchase online' or 'purchase via mail' button, respectively.
